All incoming students are required by New York State to comply with Public Health Laws 2165 and 2167 (Vaccine Requirements). These laws apply to all students, including undergraduates, graduates, transfers, commuters, residents, US citizens, International, and Canadian students.
Measles, Mumps and Rubella (New York State Public Health Law 2165)
Students born on or after Jan. 1, 1957, must submit proof of immunity to Measles, Mumps and Rubella.
- 2 vaccinations against measles and 1 vaccination for mumps and rubella are required.
- The immunizations must have been given on or after your first birthday, and at least 28 days apart.
- Your doctor’s office or your high school should have your immunization records. If you are unable to locate immunization records, your doctor can order blood testing to prove your immunity.
Meningococcal Meningitis (New York State Public Health Law 2167)
The institution is required to maintain a record of the following for each student:
- The requirement is one dose of meningococcal vaccine against strains ACYW within the past 5 years, and/or proof of a completed two or three dose vaccination series for meningococcal strain B within past 5 years.
- If you or your parent/guardian (if you are under 18 years of age) do not wish vaccination against meningococcal disease, you or parent/guardian must sign the NU Meningitis Declination form found on the Required Student Health Form page.

Physical Examination: Undergraduates, transfer, International, and Canadian students must provide a physical examination dated within one year prior to entering classes.
TB Screen: NU Tuberculosis (TB) Risk Questionnaire form required for all students.
